According to Wikipedia, "Head, Shoulders, Knees and Toes" was 1st documented in the 1950's. In the US, it's sung to the tune of "There Is a Tavern in The Town" which can first be found in print in 1883. In some places it's sung to the tune of "London Bridge is Falling Down."
There's a version in Iceland called "Höfuð herðar hné og tær". It was written by Hermann Ragnar Stefánsson (1927 - 1997).
